Please bring the following when enrolling:

• Copy of Immunization’s Record
• Emergency contact numbers
• Statement of the child’s health from a health care professional.
• Any other state requirements that’s apply. 
  
  

Tuition and Registration Fee must be paid to complete enrollment.

In accordance with the state law, you will be acquired upon admission of you child to submit a statement certifying their immunization record, a listing of any special health need, and the name of your child physician. Your child must have or be in process of obtaining all immunizations at the medically appropriate time.
